Customer Letter of Authority (CLoA) porting form explained

Current retailer

Enter the name, registered address and contact email for the provider you are porting away from. 

New retailer

Enter the name, registered address and contact email for the provider you are porting in to. 

Site address to register against numbers 

Enter the address where calls will originate from. 

Numbers to be ported (Geo and Non-Geo)

An exhaustive list of the numbers you are porting. If there is not enough room, use the continuation sheet on page 2.

MBN - Main Billing Number (If known)

The main number that appears on your bill. Other numbers and DDI ranges may be associated with this number. 

Customer's Company Details

Your company name, address and company registration number as shown on the most recent bill from the Current Retailer. Please also include your Billing Account Number for ALL types of numbers.

Requester's Details

Various details relating to the signatory. The signatory must be the end user and owner of the numbers. 

Additional Sites and/or Numbers to be Ported

Use this section if you ran out of room in the Site address to register against numbers section on page 1. Or if there are numbers which need a different originating address.

Notes 

Use this page to enter any other useful information relating to the numbers. For example, if the number has ported before and if the numbers are single or multi-line.
